   Proposal Description:
   Dementia with Lewy bodies (DLB) is the second most common cause of                neurodegenerative dementia. Overall, DLB has a more aggressive course than Alzheimer???s dementia with shorter survival and earlier time to nursing home placement. Despite the overall accelerated clinical course, a subgroup of DLB patients have a slower cognitive and respond to acetycholinesterase inhibitor treatment. The absence of coexisting AD pathology measured by neuroimaging or autopsy can identify this subgroup with a less aggressive course. Predictors of survival in DLB are currently lacking. Patients and families frequently ask for prognostic information. Most studies have focused on differences in survival between AD and DLB.                                                      The goal of our project is to identify predictors of survival among DLB           patients focusing on pathology.                                                   The primary aim of the study is to identify pathologic predictors of survival     in DLB. Based on prior work suggesting a faster cognitive decline and less treatment response with coexisting AD pathology, higher Braak neurofibrillary tangle and neuritic plaque burden may predict shorter survival from symptom onset among DLB patients. We will also evaluate whether a survival difference exists between transitional and diffuse lewy body disease.
